Non-Complex Provider

CPT 99453

Initial setup and patient education on the use of remote monitoring equipment. This is billed once per device.

CPT 99454

Monthly monitoring, which includes the supply of the device, data collection, and transmission for at least 16 days to qualify for the billing month.

CPT 99454

Monthly monitoring, which includes the supply of the device, data collection, and transmission for at least 16 days to qualify for the billing month.

CPT 99458

Additional care management services, billed in 20-minute increments, can be used in conjunction with CPT 99457.
